Community Care Choices accepts patients who:

• Have a diagnosis of a serious/chronic illness, common diagnosis include Cancer, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Dementia, Liver Disease, Renal Disease and other life challenging illnesses.
• Have a primary care physician
• Are not currently receiving hospice services
• Opting to seek/continue curative treatment
